SAEROLower Blepharoplasty: Under-Eye Fat Removal vs. Fat Repositioning
Under-eye bags can make the face appear tired, aged, or less refreshed—even after getting enough rest. While many people assume that under-eye bags are simply caused by excess fat, the actual cause may involve several factors, including protruding fat, loss of volume, skin laxity, and the natural structure of the lower eyelid.
For this reason, lower blepharoplasty should not be approached in exactly the same way for every patient. Depending on the condition of the under-eye area, the surgeon may recommend removing excess fat, repositioning existing fat, addressing loose skin, or combining more than one technique.
WHAT CAUSES UNDER-EYE BAGS?
The lower eyelid contains natural fat compartments that help support and protect the eye. As the tissues surrounding the eyes gradually weaken or change with age, these fat compartments may become more prominent and create a puffy appearance.
Genetics can also play an important role. Some patients develop noticeable under-eye bags at a relatively young age, even without significant skin laxity. Others may experience a combination of protruding fat, loose skin, fine wrinkles, and hollowing between the lower eyelid and upper cheek.
The shape of the cheekbones, the depth of the tear trough, and changes in facial volume can also affect how noticeable under-eye bags appear. Therefore, treating the fat alone may not always produce the most balanced result.
WHAT IS UNDER-EYE FAT REMOVAL?
Under-eye fat removal is a procedure that reduces a carefully selected amount of protruding fat from the lower eyelid. It may be considered when excess fat is the primary cause of puffiness and there is little or no significant hollowing beneath the under-eye bags.
The purpose is not to remove all of the natural fat. Lower eyelid fat contributes to the smooth contour and support of the eye area. If too much is removed, the area may appear hollow, sunken, or more tired over time.
For this reason, the amount and location of fat removal should be determined carefully according to the patient’s anatomy, skin condition, and overall facial proportions.
WHAT IS UNDER-EYE FAT REPOSITIONING?
Under-eye fat repositioning may be recommended when protruding fat is accompanied by a hollow tear trough or a visible groove between the lower eyelid and upper cheek.
Instead of removing most of the existing fat, the surgeon moves and redistributes it over the hollow area. This can help soften the transition between the lower eyelid and cheek while preserving natural volume.
Fat repositioning does not mean that additional fat is necessarily added from another part of the body. In many cases, the patient’s existing under-eye fat is used to create a smoother contour. However, the exact surgical method depends on the amount of available fat, the degree of hollowing, and the surrounding facial structure.
FAT REMOVAL OR FAT REPOSITIONING: WHICH IS BETTER?
Neither method is automatically better for every patient.
Fat removal may be appropriate when puffiness is the main concern and the area beneath the eye already has sufficient volume. Fat repositioning may be more suitable when under-eye bags and hollowness appear together.
Some patients may benefit from a combination of conservative fat removal and fat repositioning. Patients with significant skin laxity or fine wrinkles may also require treatment of the lower eyelid skin rather than fat correction alone.
Choosing the appropriate method requires an evaluation of the entire lower eye area, including fat distribution, tear-trough depth, skin elasticity, eyelid support, and the relationship between the lower eyelid and cheek.
WHERE IS THE INCISION MADE?
The incision location depends on the patient’s condition and the surgical plan.
For patients with good skin elasticity and no significant excess skin, the procedure may be performed through the inside of the lower eyelid. This is known as a transconjunctival approach and does not leave an external incision on the skin.
When loose or excess skin also needs to be addressed, an incision may be placed just beneath the lower eyelashes. Through this approach, the surgeon can carefully manage the fat and skin while positioning the incision close to the natural eyelid contour.
The most appropriate incision method is selected after considering the patient’s age, skin condition, eyelid structure, and treatment goals.
CAN LOWER BLEPHAROPLASTY IMPROVE DARK CIRCLES?
Lower blepharoplasty may improve certain types of dark circles, particularly when they are caused by shadows created by protruding fat or a deep tear trough. Smoothing the contour beneath the eyes can reduce these shadows and create a brighter, more refreshed appearance.
However, not all dark circles are caused by under-eye bags. Pigmentation, thin skin, visible blood vessels, allergies, and individual facial anatomy can also contribute to discoloration.
If pigmentation is the primary cause, lower eyelid surgery alone may not completely resolve the concern. A detailed consultation is necessary to identify the main cause and establish realistic expectations.
WHAT IS RECOVERY LIKE?
Swelling, bruising, tightness, and temporary discomfort may occur during the early recovery period. Some patients may also experience temporary dryness, irritation, or sensitivity around the eyes.
These symptoms generally improve gradually, although the pace of recovery varies depending on the surgical method and the individual. Residual swelling may remain even after the patient feels comfortable returning to normal daily activities, and the final contour becomes more refined as the tissues settle.
Patients should carefully follow all post-operative instructions, use prescribed medication as directed, avoid rubbing or placing pressure on the eyes, and attend scheduled follow-up appointments. The surgeon should provide individualized guidance regarding makeup, contact lenses, exercise, and other activities.
WHO MAY CONSIDER LOWER BLEPHAROPLASTY?
Lower blepharoplasty may be considered by patients who are concerned about persistent under-eye puffiness, prominent fat bags, a deep tear trough, loose lower eyelid skin, or a tired-looking appearance that does not improve with rest.
However, the procedure must be carefully planned because the lower eyelid is a delicate area, and every patient has a different combination of fat, skin, muscle, and facial volume.
The goal is not simply to remove as much fat as possible. A well-planned procedure should create a smoother transition between the lower eyelid and cheek while maintaining natural volume, eyelid shape, and overall facial balance.
At SAERO Plastic Surgery, lower blepharoplasty is individually planned according to each patient’s under-eye anatomy, fat distribution, skin condition, facial structure, and aesthetic goals. A detailed consultation is recommended to determine whether fat removal, fat repositioning, skin correction, or a combined approach may be most appropriate.
